Empowering Independence Through Diverse Support Programs

We collaborate across multiple programs to provide the resources that promote independence, inclusion, and community. Unlimited Care offers services through the following programs.

ACT 150 Waiver

The ACT 150 Waiver is a Pennsylvania program that provides home and community-based services for adults with physical disabilities. It helps participants live independently in their own homes by offering support tailored to their individual needs.

You may qualify if you:

  • Are 18 to 59 years old
  • Are a resident of Pennsylvania
  • Are capable of directing your own care and managing your finances and legal affairs
  • Have a medically determinable physical impairment that is expected to last at least 12 months or may result in death
  • Understand that a minimal monthly co-payment may apply

Area Agency on Aging (AAA) OPTIONS Program

Programs designed to help older adults maintain independence, safety, and quality of life. Services may include: personal care, respite, homemaking, and community supports.

You may qualify if you:

  • Are 60 years or older
  • Live in Pennsylvania
  • Are a U.S. citizen or legal resident
  • Need help with daily activities at home
  • Live in a private home or community setting
  • Understand that income does not affect eligibility (some may have a small cost share)

Medical Assistance Transportation Program (MATP)

MATP provides transportation assistance for individuals with Medical Assistance who need help getting to and from medical appointments. This program ensures that eligible individuals can access essential healthcare services safely and reliably.

You qualify if:

You need transportation to receive covered medical services — and you have no other reasonable transportation available.

  • You have a valid Pennsylvania Medical Assistance (Medicaid) / ACCESS card
  • You live in a county served by MATP
  • You need transportation to receive covered medical services — and you have no other reasonable transportation available.

Community HealthChoices (CHC)

CHC enables individuals to receive home and community-based services, helping them remain independent and active in their communities. Services may include: personal assistance, community supports, daily living help, transportation, and more.

You may qualify for CHC if:

  • You are 21 years old or older, and
  • You are either:
    • Dually eligible for both Medicare and Medicaid, or
    • Qualify for Medicaid long-term services and supports (LTSS) — meaning you need a level of care similar to what’s provided in a nursing facility (but may receive supports at home)

Veteran Services

Dedicated assistance to veterans and surviving spouses needing in-home or community-based support to enhance independence and wellbeing. The VA program provides support for Veterans who need help with daily activities or personal care. It is also available for Veterans who are isolated or whose caregiver is experiencing caregiver burden.

Homemaker and Home Health Aide services can:

  • Be used alongside other Home and Community-Based Services
  • Serve as an alternative to nursing home care
  • Provide respite care for Veterans and their family caregivers

These services help Veterans of any age remain living safely and independently in their own home.

Office of Developmental Programs (ODP)

Empowering individuals with intellectual and developmental disabilities through person-centered supports. Services may include: community participation, in-home supports, group home services, companion services, skill-building and transportation.

You may qualify if you:

  • Have an intellectual disability, autism, or developmental disability
  • Had the disability before age 22
  • Need support with everyday life skills
  • Meet Medical Assistance (Medicaid) requirements for certain programs
